The Wellness Rewards Program is offered to all Juniata College Employees and is a great way to stay fit and to earn prizes for the healthy choices you make!

Rewards program members receive rewards points for many types of activities including regular exercise, preventive screenings, community service, weight loss programs, injury prevention, participation in Wellness Committee events, and attendance at the annual health fair just to name a few.  All you need to do is submit a monthly activity form documenting your activities. 

Each quarter you will be notified of the total points you have accumulated during that period, so that you may keep track of progress toward your reward. Rewards will be distributed in December or January, depending on your selection of stipend or vacation day.  Turn in your logs monthly, and allow your points to accrue for the top prize. Please note that points do not carry over from year to year.

All logs must be received in Human Resources by the 10th of each month to be counted - no exceptions! To participate, complete the registration form below and return it to Human Resources.

Disclaimer:    Should your employment from Juniata be terminated (either voluntarily or involuntarily) during the course of the year, your Wellness Rewards earnings will be forfeited.   You must be employed by Juniata through December 31st in order to redeem your Wellness Rewards points.
